Pinnacle Family Health, Inc. is a private, non-profit, 501(c)(3) corporation located in West Monroe, Louisiana. On November 01, 2019, we were excited to become designated by the United States Department of Health and Human Services (HHS) Health Resources and Services Administration (HRSA) Bureau of Primary Health Care (BPHC) as a Federally Qualified Health Center Look-Alike (more commonly known as a Community Health Center). What does this mean for the people in Northeast Louisiana? It means we are able to help you reach the Peak of Healthcare by providing affordable and comprehensive healthcare. We’re run by a board of directors, most of whom are patients of Pinnacle Family Health. We exist to serve people who have limited access to healthcare, including people who are low-income, uninsured or underinsured; however, anyone who wants to receive their healthcare in an affirming and inclusive environment is welcome at Pinnacle Family Health!
We want people to feel like they're being welcomed into our home, which is why we strive to be the place where youcan come to openly communicate with your healthcare provider so you can be given the highest level of care and so we can work together in helping you live your healthiest life.
Pinnacle Family Health offers services based on a sliding fee scale for people who are low-income and uninsured or underinsured, thereby providing a means to remove the cost barrier that otherwise prevents many people from seeking the medical or behavioral health care they need and deserve.
Pinnacle Family Health receives grant funds and/or operates under programs administered by our partners below. We also receive private donations to help make our mission and vision a reality. Our current partners include: (1) the Health Center Program of the Bureau of Primary Health Care and (2) the Ryan White HIV/AIDS Program, both of the Health Resources and Services Administration (HRSA) of the U.S. Department of Health and Human Services (DHHS); (3) the STD/HIV Program of the Louisiana Department of Health; (4) Broadway Cares Equity Fights AIDS; and (5) the Living Well Foundation.
